He went by his birth surname, Gleistein, until 1930, when it changed to Gleiston. At least some members of his family changed their surname to Gilbert to match his actress daughter's stage name. I don't know if the names were legally changed or not. His daughter Florence's California Death Index entry shows her father's surname was Gilbert, which was never the case.

I don't know if he is inurned at Chapel of the Pines or not, but his d/c does say Los Angeles Crematory, which was the official name for Chapel of the Pines. So he was at least cremated there whether a niche was purchased or not.
